Cindy started Community Horizons LLC with the goal of helping others overcome the barriers to living a self-directed life. With a background in human services and human resources, she hopes to play an active role in helping you achieve your goals through additional advocacy and support.

Cindy has worked within the IRIS program and holds over 14 years of experience with employee relations and the recruitment of caregivers. When working with Cindy you can expect connection and coordination with compassion.

Joyce is personable and action oriented with a strong ability to communicate efficiently. Driven by passion and commitment Joyce takes pride in providing the best assistance to every individual she supports.

Her passion for helping others drives with a background in EMS-I/Fire Fighter along with 23 years in healthcare as a Program Director and Division Administrator. Joyce has had the ability to gain great knowledge and build community relationships to assist in successful outcomes

Angela is caring, compassionate, and dedicated to helping others succeed as well as an advocate for every individual she works with. She grew up in Merrill and currently is local in Wausau.Angela gained her experience throughout her healthcare career as a CNA, manager, and recruiter. Angela has had the pleasure of working with many community supports and programs. She has built great long-lasting relationships with individual members, families, and care teams. Angela is currently accepting referrals in Marathon and Lincoln Counties

Pam is hardworking, caring, compassionate, and a strong advocate to ensure member’s goals are met. With over 20 years of experience working within the healthcare field, she has developed strong, long-lasting relationships with members, care teams, guardians, and family members. Pam is accepting referrals in Marathon, Lincoln, Portage and Shawano counties.

 Abby is positive and goal-oriented with a passion for helping others succeed to meet their goals. She was born and raised in Green Bay and has over 10 years of experience. She started in the field as a caregiver and worked her way into leadership and recruiting roles within the Human Services field. She demonstrates compassion for everyone and firmly believes in self-advocacy. Abby looks forward to building a strong rapport with community members in assisting individuals whether it may be for recruiting or housing needs. Abby is currently taking referrals in Brown and Door counties. 

 Jaime is currently taking referrals in Outagaime and Winnebago counties.
